Eternal Hearts replaces one of your red hearts temporarily. If you successfully end a level, you gain your heart back, plus the new one as in the normal mode.

Item appearance rate?[]

Obviously it's hard to guaranteed confirm this, but contrasting videos of people playing on Normal with my experiences on Hard, it appears the general item drop rate (such as hearts, keys, etc. appearing in the room after clearing all the enemies from it) is absurdly small in Hard. I've literally gone the entire Caves II without a single item dropping from a room, and this is a persistent experience. I frequently don't have a key on hand to unlock the treasure room on each floor. --GarlyleWilds (talk) 21:04, 5 November 2014 (UTC)

Are there any benefits from playing in hard mode? I'm sure I read somewhere that some items only drop in Hard Mode. --77.102.114.99 21:38, 29 December 2014 (UTC)

I believe you mean that some items are unlocked in Hard Mode. That'd be Godhead, unlocked by completing The Lost's post-it note (Major bosses + Boss Rush) all in Hard mode. Also, every character has a specific co-op baby that is unlocked by defeating Mom's Heart/It Lives on Hard Mode, some special babies, some just graphically different. I believe said babies are required for Real Platgod.

Here's what you can unlock with Heard Mode: Each character unlocks a special Co-Op Baby for beating mom's heart in hard mode. If you beat ???, The Lamb and Mega Satan with The Lost on Hard Mode you unlock Godhead. Except for Godhead whihc is required for Real Platinum God (but not Platinum God alone), no other unlock is required for any "XXX God" achievement. I might add that here on the page eventually.

Micro battery replacement[]

In the patch notes, the addition of micro batteries (the ones that fill 2 charges) was in a section dedicated to hard mode changes, so I assumed that meant they were exclusive to hard mode. They aren't, but I would still assume that they might have a higher replacement rate for Lil' Batteries than in Normal Mode. Confirmation for that would be great.
