Tainted Lost is a character added in The Binding of Isaac: Repentance. It is the alternate version of The Lost. Tainted Lost is unlocked by reaching Home and using the Red Key, Cracked Key, or to access a special room in one of the walls while playing as the Lost.

Tainted Lost is just like The Lost but instead of Holy Mantle, it starts with Holy Card and does not start with the Eternal D6. Tainted Lost has flight and spectral tears, but no health and dies from any damage taken. Cards and Runes have a 10% chance of being turned into a Holy Card.

Tainted Lost is just like The Lost, but instead of Holy Mantle, it starts with a Holy Card; it also does not start with the Eternal D6. Like the Lost, Tainted Lost has flight and spectral tears, but no health. Just as the Holy Mantle effect from the Holy Card negates damage taken in the first place, items such as Birthright, Missing Poster and Soul of Lazarus will revive Tainted Lost after death. This means that items that normally take effect upon taking damage will affect Tainted Lost upon death and revival. It also starts with a 1.3x damage multiplier, giving it higher starting damage and making most damage ups more powerful. Certain items will be automatically rerolled while playing as Tainted Lost, including most defense-oriented items (see Item Exclusion). To offset the lack of Holy Mantle and limited defensive options, all cards that spawn have roughly a 10% chance of being turned into a Holy Card.

Beating Delirium with the The Lost is not required for Tainted Lost to obtain the Holy Card.
+
Beating Delirium with The Lost is not required for Tainted Lost to obtain the Holy Card, or for Holy Cards to spawn during the run.

  +
As with The Lost, Devil Room items and Black Market items can be taken for free, but will be presented as a choice. Taking one item as a deal will cause the others in the room to disappear.
Item Quality Bonus
Tainted Lost has an additional mechanic which prevents unusable and low-quality items from spawning, such as items that only provide health increases (which Tainted Lost is unable to use)

  +
Item Exclusion
Categorization and quality are hidden values assigned to each item in the game. If an item that does not qualify would be generated, it's instead rerolled into a suitable item.

  +
Tainted Lost has two mechanics that determine what items it can encounter.

  +
Offensive items
Additionally, Tainted Lost has a 20% chance of rerolling any item with a quality value of 2 or less – such as Brother Bobby or Almond Milk – even if they would otherwise qualify.

  +
The first mechanic makes it so that any items that are not tagged "<code>offensive</code>" cannot be naturally encountered while playing as Tainted Lost. This disallows items that it can't take advantage of, similar to The Lost's Birthright, but it also prevents many powerful defensive items, such as Dead Cat and Holy Mantle, from naturally spawning. These items will be rerolled, and items that are set drops from mini-bosses (ex. Super Pride dropping Dead Cat) will never be dropped.

  +
Disallowed items can only appear by using Spindown Dice, Death Certificate, as set drops from props (ex. Dry Baby from a skull), or from Soul of Lilith.
This, unfortunately, precludes items such as Dead Cat and Holy Mantle. These cannot appear in game naturally (but will still appear through methods and situations that forcibly assign an item of a specific ID to spawn, such as Spindown Dice).

  +
Not all items tagged "<code>offensive</code>" are particularly offense-oriented items. Examples of this include Treasure Map and The D6.
Excluded Items
  +
  +
This mechanic seemingly does not work correctly in co-op.
  +

Item quality
  +
The second mechanic uses the item quality system. Items with a quality value of 2 or less have a 20% chance of being rerolled when encountered as Tainted Lost. The new item chosen is not guaranteed to have quality over 2.
  +

Excluded Items
The following items cannot be found while playing as Tainted Lost.

Strategy
Strategy
With only a single Holy Card for defense, defensive items become just as important as offensive ones, as you won't be able to afford taking more than a single hit the entire run unless you find another Holy Card. However, because all items not marked as "offensive" will automatically be rerolled, defensive options available are less plentiful.

With only a single Holy Card for defense, defensive items become just as important as offensive ones, as taking more than a single hit the entire run may prove fatal. However, because all items not marked as "offensive" will automatically be rerolled, defensive options available are less plentiful.
  +
  +
Tainted Lost starts with a Holy Card regardless of whether the Holy Card is unlocked. Tainted Lost can find Holy Cards even if it is not unlocked by defeating Delirium as The Lost.
  +
  +
* A good way to secure a powerful run is to visit as many Boss Challenge Rooms as possible. It is possible for the player to use the alternate floors to their advantage by sticking to the second floor of each chapter. By visiting Downpour II / Dross II, the player can also get an extra boss item from the Mirrored World. This strategy comes with the added risk of having to fight more bosses, but provides the potential to snowball the player's stats and make later floors much easier to survive in.
  +
** Remember to fight the boss outside the Mirrored World, which will drop Perfection.
  +
* Be careful of any , which could be a or a .
  +
* Skulls can be a source of finding cards which in turn can be a Holy Card.
  +
* Waiting before using the Holy Card until after the Shop has been found in case a Blank Card is available for sale can prove helpful.
  +
* Be wary of taking items that automatically attack enemies in case they kill exploding enemies nearby. Items like 120 Volt, Demon Baby and items that spawn blue flies.

Good Items
Good Items
  +
These items provide significant benefits to Tainted Lost.
* 2spooky: The fear aura makes contact damage from enemies almost impossible to take

  +

* Ace of Spades: Chance of changing the room drop reward to a card, and chance of getting a card as additional rewards from certain things, indirectly increasing the chance of getting a Holy Card.

* Ace of Spades: Chance of changing the room drop reward to a card, and chance of getting a card as additional rewards from certain things, indirectly increasing the chance of getting a Holy Card.

* Birthright: Grants an extra life, respawning in the same room and damaging nearby enemies.
* Anima Sola

  +
** This extra life item is the only one that can be naturally obtained as Tainted Lost, as all others are rerolled.
* Birthright: Grants an extra life, respawning in the same room and damaging enemies in an area.

  +
** Taking a hit and using Birthright counts as taking damage and will affect items accordingly, such as losing Perfection.
* Blank Card: Allows reuse of the Holy Card.

  +
** Summoning Birthright with Lemegeton will grant infinite lives until the wisp is destroyed.
* Blanket: One-time Holy Mantle for boss battles.

* Blank Card: Allows a four charge reuse of the Holy Card.
* Bloodshot Eye

  +
** 9 Volt and AAA Battery each reduce the charge time required by one bar.
* Book of Shadows: Very powerful in combination with Panic Button, and allows infinite invincibility when fighting most endgame bosses if the player holds 4.5 Volt.

  +
** The Battery allows storing an extra use of Blank Card, useful in case you take a hit multiple times in a short period.
* Book of the Dead: Bone orbitals.

  +
* Bone Spurs: Especially useful against enemies that spawn bullets or other enemies on death, as they will often be instantly blocked or damaged/killed, respectively.
* Book of Virtues

* Book of Shadows: Very powerful in combination with Panic Button and 4.5 Volt.
* Broken Modem: May delete enemy shots at random, in addition to its other bonuses.

  +
* Book of the Dead: The Bone orbitals spawned from this item can block projectiles, greatly increasing survivability.
* Camo Undies: Allows Isaac to be invisible upon entering a room and plan out their first attack.

  +
* Book of Virtues / Lemegeton: Rings of wisps can block a lot of projectiles.
* Censer: Slows projectiles to make them easier to dodge.

  +
** Summoning the Birthright wisp with Lemegeton will grant infinite lives until the wisp is destroyed.
* Clear Rune: Can be used with Rune of Algiz.

  +
* Booster Pack: Has about a 50% chance of spawning a Holy Card.

* Camo Undies: Allows Tainted Lost to be invisible upon entering a room and plan out his first attack.
  +
* Clear Rune: Can be used with protective runes like Rune of Algiz or . Similarly, Soul of the lost can be used for a single-use Holy mantle for the current room.
  +
** Soul of Lazarus also provides a short invincibility streak after reviving.
  +
* Clicker: Becoming Tainted Lost by using Clicker allows Tainted Lost to keep all previously collected items. This allows many useful defensive items that Tainted Lost normally can't collect.

* Coupon: Allows an extra item to be taken from devil deals.

* Coupon: Allows an extra item to be taken from devil deals.
  +
* Cursed Eye: The downside of the item is inconsequential due to Tainted Lost's lack of health.
* D1: Allows duplicating Holy Cards.

  +
* D1 / D infinity /Dice Bag: Cards duplicated by D1's effect have an increased chance to be Holy Cards.
* Dad's Ring

* Damocles: The downside of the item is inconsequential due to Tainted Lost's lack of health.

* Damocles: The downside of the item is inconsequential due to Tainted Lost's lack of health.
  +
** The item can still kill Tainted Lost if resurrected by Birthright or any other acquired respawn item.
* Dark Arts: The temporary invincibility combined with the high damage output makes this a really good option.

  +
* Dark Arts: Provides a powerful panic button to escape swarms of enemies and/or bullets.
* Deck of Cards: Allows more chances of Holy Cards.

* Echo Chamber: Allows every consumable to be a Holy Card if one is used after obtaining.
+
* Deck of Cards: Allows more chances for Holy Cards.
  +
* Death Certificate: Tainted Lost can now obtain defensive items, as Death Certificate's item generation ignores Tainted Lost's changes to item availability. This is extremely beneficial, as Tainted Lost can now obtain extra lives such as Dead Cat and protective items such as Holy Mantle.
  +
* Echo Chamber: The next three consumables used after Holy Card will grant another mantle.
  +
* Endless Nameless: Has a 25% chance to spawn another Holy Card when one is used.
  +
* Flat File: Retracts almost all spikes and allows free Curse Room entrance.

* The Gamekid / My Little Unicorn: On-demand invincibility with contact damage.

* The Gamekid / My Little Unicorn: On-demand invincibility with contact damage.
  +
* Glass Cannon: High damage shots that allow removal of especially dangerous enemies fast. The downside of the item only comes into effect upon use of a reviving item and not from consuming a Holy Mantle.
* Any luck-based tear effect such as Apple, Tough Love, Holy Light, Euthanasia, Ghost Pepper and Bird's Eye benefits well from Perfection. One of the most powerful tear effect among them is Ghost Pepper: at 11 luck, every shot is accompanied by a blue spectral flame that deals heavy damage and blocks enemy projectiles.

* Glowing Hour Glass: Allows recovery of a Holy Card shield.
+
* : Bombs may drop a Holy Card when exploding.

* Glowing Hourglass: Allows recovery of a Holy Card shield.
* Lost Contact

  +
* Guppy's Tail: can increase the amount of Locked Chests found in a run, and thus allows more chances for Holy Cards to appear.
  +
* Homing tears: Allows Tainted Lost to safely position himself almost anywhere in a room while still dealing damage.
  +
* Little Horn / Euthanasia: can destroy almost all "invulnerable" enemies (e.g. Poky, Stone Grimace) before they can hurt Tainted Lost. However, Ball and Chain are still invulnerable and dangerous.

* Lost Contact: Blocks projectiles.

* VI - The Lovers?: Can be safely used to generate items with no loss of durability or risk of death.

* VI - The Lovers?: Can be safely used to generate items with no loss of durability or risk of death.
* Mercurius: Most rooms can be checked for a safe plan of attack, or bypassed entirely.
+
* Mercurius / Door Stop: Most rooms can be checked for a safe plan of attack, or bypassed entirely.
* Missing Poster: Allows a one-time revive upon death, respawning Tainted Lost in the previous room.
+
* Missing Poster: Allows a one-time revival upon death, respawning Tainted Lost in the previous room.
  +
** Mysterious Paper has a chance to trigger Missing Poster's effect each room.
* Paschal Candle: Lost's lack of health makes it stay at maximum level.

  +
* : Gets strong Trinkets and doubles their effect.
* Pause: Can be used with a damaging orbital such as Cube Of Meat, or Ball Of Bandages to get lots of damage on bosses for free

  +
** Broken Remote could be used as Teleport 2.0, which also allow Tainted Lost to leave Curse Room safely.
* Perfection: 10 luck with no downside as you die anyway if you get hit.

  +
* : Could save Tainted Lost from death when combining with suitable active items.

* Paschal Candle: Taking a hit and consuming a Holy Mantle effect allows it to stay at maximum level.

* Pause: Can be used with a damaging orbital to get lots of damage on bosses for free.
  +
* Perfection: +10 luck with no downside. Tainted Lost's lack of health make losing it during the run less likely as only using a reviving item will consume Perfection. Luck-based tear effects such as Apple!, Tough Love, Holy Light, and Euthanasia benefit greatly from this extra luck, while also making sure Tainted Lost has a steady supply of pickups from room clear rewards.
  +
** Bird's Eye and Ghost Pepper are exceptionally powerful combined with high luck: at 10 luck, every tear has a 50% chance to be accompanied by a powerful flame that blocks enemy projectiles. This becomes even more potent when both items are held, as all tears will fire one of these two flames at only 7 luck.

* Placebo: Protective pills like Power Pill or Feels Like I'm Walking on Sunshine may be helpful.

* Placebo: Protective pills like Power Pill or Feels Like I'm Walking on Sunshine may be helpful.

* Pluto: Smaller size makes you harder to hit.

* Pluto: Smaller size makes you harder to hit.
  +
* Soul of Lazarus: Acts as a substitute to Holy Card at the cost of losing Perfection.
* Psy Fly: Blocks many projectiles from hitting Isaac.

  +
* Spindown Dice: Allows Tainted Lost to obtain items otherwise hard-coded to not spawn for them.
* Slipped Rib: Reflects projectiles making them not hit Isaac.

  +
** Some very useful reroll chains for Tainted Lost include:
* Starter Deck: Increases the chances of finding Holy Cards, and allows pill-generating items to spawn Holy Cards.

  +
*** Belly Button to Cone Head
* The Soul: Slight projectile reflection aura makes it easier to dodge things.

  +
*** Book of Secrets to Blank Card
* The Stairway: Creates a ladder to an Angel Room shop at the start of every floor, with the shop potentially selling powerful Angel Room Item Pool items and Holy Cards.

  +
*** Cricket's body to Pyromaniac
* Telekinesis

  +
*** Free Lemonade to Damocles
* Tooth and Nail: Timed invincibility allows exiting Curse Rooms safely.

  +
*** GB bug to Farting Baby
* Wooden Cross: A single-use Holy Mantle per floor. Note that it does not stack with Holy Card; it is thus wise to save Holy Cards for when the shield from the Wooden Cross is broken.

  +
*** Juicy Sack to Dry Baby
  +
*** Magic Mushroom to 1up!
  +
*** Meat Cleaver to Bot Fly to Death Certificate
  +
*** Restock to Host Hat
  +
*** Strange Attractor to Thunder Thighs to Holy Mantle
  +
*** Tech.5 to Trinity Shield to Infamy
  +
*** Void to D1
  +
*** We Need To Go Deeper! to The Nail to Lord of the Pit to Dead Cat
  +
*** Lost Contact to Guppy's Collar to Spiderbaby to Gnawed Leaf
  +
*** to to

* Starter Deck: Increases the chances of finding Holy Cards and allows pill-generating items to spawn Holy Cards.
  +
* Teleporting cards: Allows exiting Curse Rooms safely.

* The Stairway: Creates a ladder to an Angel Room shop at the start of every floor, with the shop potentially selling powerful Angel Room Item Pool items and Holy Cards.
  +
* Telekinesis: Can be used to block projectiles or enemies.

* Tooth and Nail: Timed invincibility allows exiting Curse Rooms safely.
  +
* Uranus: Frozen enemies will not be able to spawn hazards (tears, explosives, other enemies) on death. Icicles from frozen enemies can also slow other enemies and potentially cause a chain reaction, freezing a whole group of enemies.

* Wooden Cross: A single-use Holy Mantle per floor. Note that it does not break before Holy Card, thus it is wise to save them for when the shield from Wooden Cross is broken.
  +
  +
Bad Items
  +
These items provide significant drawbacks to Tainted Lost.
  +
  +
* Any explosive tear effect, such as Fire Mind or Ipecac, greatly increases the chance of fatal self-damage. Since Host Hat and Pyromaniac cannot naturally appear as Tainted Lost, there are very few ways to reliably avoid explosive self-damage.
  +
** The same risk also applies for explosive familiars like BBF, Bob's Brain, and Lil Delirium.
  +
** 3 Dollar Bill, Fruit Cake, and Playdough Cookie have a chance to shoot Fire Mind tears.
  +
* Tear effects that destroy rocks (Sulfuric Acid, Terra) and the Stompy effect make firing from cover impossible – all but removing a safe method for Tainted Lost to attack non-flying enemies.
  +
* : May destroy TNT accidently.
  +
* Little Baggy: Holy Cards will no longer spawn.
  +
* IBS: The effects of poop caused by damaging enemies is completely random, which can lead to an accidental death or mantle loss.
  +
** Butt bombs may hurt you if you aren't paying attention, gas clouds from stinky poop could explode in a chain back to you if thrown at fires, and farts generate a gas cloud that will explode if you are next to a fire, leading to an untimely death.
  +
  +
Neutral Items
  +
These items can provide both benefits and drawbacks to Tainted Lost.
  +
  +
* Although items which provide temporary invincibility such as A Pony and Mars can allow Tainted Lost to negate damage, their effects are difficult to time, and could result in charging into an enemy and being damaged.
  +
* Buddy in a Box: The familiar's potential tear effects are the same as Fruit Cake, and as such have a small risk of carrying Fire Mind for a floor.
  +
* Dead Sea Scrolls: Can cause items such as Anarchist Cookbook to trigger, potentially causing damage, but may also spawn a Holy Card via Deck of Cards.
  +
* Experimental Treatment: HP is not a factor for Tainted Lost, so it may be included as one of the two stats which are reduced. However, it is more likely to be included as a stat increase, which has no effect.
  +
* Lemegeton: Can give Tainted Lost very useful items, such as Birthright, allowing him to have infinite lives until the wisp is destroyed, but it can also give run-ending items such as Ipecac. Because Lemegeton's effect is random, this is completely out of the player's control. If a very bad wisp is gained using Lemegeton, it is recommended to destroy it as soon as possible, but be cautious as this could result in taking damage.
  +
* Monster Manual: Can spawn Bob's Brain or Lil Delirium, which may cause unavoidable damage to the player. However, it can spawn many useful familiars too.
  +
* Number One: Decreasing maximum range exposes Tainted Lost to danger, but increasing fire rate may make it easier to defeat enemies.
  +
* Teleport! / Teleport 2.0: Can teleport Tainted Lost away from a dangerous room or before a fatal mistake is made. However, the player can teleport to a Curse Room (or to a Secret Room whose only opened exit is a Curse Room) which is inescapable without a Holy Card.

** "Baleful" means "ominous" or "harmful or malignant in intent or effect", possibly referring to how Tainted Lost has higher base damage than his regular counterpart and can find better items.
  +
** A more archaic meaning of "Baleful" is "Something/Someone wretched or miserable", and considering how Tainted Lost is incapable of using defensive heavy builds, being constantly at the mercy of enemies and dying usually at two hits, fit with this use of the word.

** "The Baleful" is Tainted Lost's character name in Four Souls.

  +
** The Fool's numeration can be either 0 the lowest trump or 22 the highest trump, linked to his attributes — better items, no mantle, no health — as Tainted Lost will die in a single hit at any point, but can also build the strongest runs of any character because of those same attributes.
  +
** Reverse fool means apathy and nullity; this approach is often recommended to face Tainted Lost's runs because of how demanding the character is and how easy it is to lose these runs despite how powerful the character can become.
  +
** In a literal reversal of "the beginning of the journey" which The Fool represents, Tainted Lost represents the complete end of Isaac's existence, after his own mortal remains has vanished and even his own soul erodes into dust and cobwebs along with the house and material objects that hold his last remnant memories.
  +
*Tainted Lost's starting health is listed as "?" on the character selection screen.

(in Repentance)

Tainted Lost is a character added in The Binding of Isaac: Repentance. It is the alternate version of Character The Lost iconThe Lost. Tainted Lost is unlocked by reaching HomeHome and using the Collectible Red Key iconRed Key, Cracked KeyCracked Key, or Soul of CainSoul of Cain to access a special room in one of the walls while playing as the Lost.

Tainted Lost is just like The Lost, but instead of Collectible Holy Mantle iconHoly Mantle, it starts with a Holy CardHoly Card; it also does not start with the Collectible Eternal D6 iconEternal D6. Like the Lost, Tainted Lost has flight and spectral tears, but no health. Just as the Holy Mantle effect from the Holy Card negates damage taken in the first place, items such as Collectible Birthright iconBirthright, Missing PosterMissing Poster and Soul of LazarusSoul of Lazarus will revive Tainted Lost after death. To offset the lack of Collectible Holy Mantle iconHoly Mantle and limited defensive options, all cards that spawn have roughly a 10% chance of being turned into a Holy CardHoly Card.

Beating Delirium with Character The Lost iconThe Lost is not required for Tainted Lost to obtain the Holy CardHoly Card, or for Holy Cards to spawn during the run.

As with The Lost, Devil RoomDevil Room items and Black MarketBlack Market items can be taken for free, but will be presented as a choice. Taking one item as a deal will cause the others in the room to disappear.

Item Exclusion[ | ]

Tainted Lost has two mechanics that determine what items it can encounter.

Offensive items[ | ]

The first mechanic makes it so that any items that are not tagged "offensive" cannot be naturally encountered while playing as Tainted Lost. This disallows items that it can't take advantage of, similar to Character The Lost iconThe Lost's Collectible Birthright iconBirthright, but it also prevents many powerful defensive items, such as Collectible Dead Cat iconDead Cat and Collectible Holy Mantle iconHoly Mantle, from naturally spawning. These items will be rerolled, and items that are set drops from mini-bosses (ex. Super Pride dropping Collectible Dead Cat iconDead Cat) will never be dropped.

Disallowed items can only appear by using Collectible Spindown Dice iconSpindown Dice, Collectible Death Certificate iconDeath Certificate, as set drops from props (ex. Collectible Dry Baby iconDry Baby from a skull), or from Soul of LilithSoul of Lilith.

Not all items tagged "offensive" are particularly offense-oriented items. Examples of this include Collectible Treasure Map iconTreasure Map and Collectible The D6 iconThe D6.

This mechanic seemingly does not work correctly in co-op.

Item quality[ | ]

The second mechanic uses the item quality system. Items with a quality value of 2 or less have a 20% chance of being rerolled when encountered as Tainted Lost. The new item chosen is not guaranteed to have quality over 2.

Strategy[ | ]

With only a single Holy CardHoly Card for defense, defensive items become just as important as offensive ones, as taking more than a single hit the entire run may prove fatal. However, because all items not marked as "offensive" will automatically be rerolled, defensive options available are less plentiful.

Tainted Lost starts with a Holy Card regardless of whether the Holy Card is unlocked. Tainted Lost can find Holy Cards even if it is not unlocked by defeating Delirium as The Lost.

  • A good way to secure a powerful run is to visit as many Boss Challenge RoomBoss Challenge Rooms as possible. It is possible for the player to use the alternate floors to their advantage by sticking to the second floor of each chapter. By visiting DownpourDownpour II / DrossDross II, the player can also get an extra boss item from the Mirrored World. This strategy comes with the added risk of having to fight more bosses, but provides the potential to snowball the player's stats and make later floors much easier to survive in.
    • Remember to fight the boss outside the Mirrored World, which will drop PerfectionPerfection.
  • Be careful of any ChestChests, which could be a Mimic chest RepentanceMimic Chest or a Haunted ChestHaunted Chest.
  • Skulls can be a source of finding cards which in turn can be a Holy CardHoly Card.
  • Waiting before using the Holy CardHoly Card until after the Shop has been found in case a Collectible Blank Card iconBlank Card is available for sale can prove helpful.
  • Be wary of taking items that automatically attack enemies in case they kill exploding enemies nearby. Items like Collectible 120 Volt icon120 Volt, Collectible Demon Baby iconDemon Baby and items that spawn blue flies.

Good Items[ | ]

  • Collectible Clicker iconClicker: Becoming Tainted Lost by using Clicker allows Tainted Lost to keep all previously collected items. This allows many useful defensive items that Tainted Lost normally can't collect.
  • Collectible Coupon iconCoupon: Allows an extra item to be taken from devil deals.
  • Collectible Cursed Eye iconCursed Eye: The downside of the item is inconsequential due to Tainted Lost's lack of health.
  • Collectible D1 iconD1 / Collectible D infinity iconD infinity /Dice BagDice Bag: Cards duplicated by D1's effect have an increased chance to be Holy Cards.
  • Collectible Damocles iconDamocles: The downside of the item is inconsequential due to Tainted Lost's lack of health.

  • Collectible Glitter Bombs iconGlitter Bombs: Bombs may drop a Holy Card when exploding.
  • Collectible Glowing Hourglass iconGlowing Hourglass: Allows recovery of a Holy Card shield.
  • Collectible Guppy's Tail iconGuppy's Tail: can increase the amount of Locked ChestLocked Chests found in a run, and thus allows more chances for Holy Cards to appear.
  • Homing tears: Allows Tainted Lost to safely position himself almost anywhere in a room while still dealing damage.
  • Collectible Little Horn iconLittle Horn / Collectible Euthanasia iconEuthanasia: can destroy almost all "invulnerable" enemies (e.g. PokyPoky, Stone GrimaceStone Grimace) before they can hurt Tainted Lost. However, Ball and ChainBall and Chain are still invulnerable and dangerous.

  • Collectible Pause iconPause: Can be used with a damaging orbital to get lots of damage on bosses for free.
  • PerfectionPerfection: +10 luck with no downside. Tainted Lost's lack of health make losing it during the run less likely as only using a reviving item will consume Perfection. Luck-based tear effects such as Collectible Apple! iconApple!, Collectible Tough Love iconTough Love, Collectible Holy Light iconHoly Light, and Collectible Euthanasia iconEuthanasia benefit greatly from this extra luck, while also making sure Tainted Lost has a steady supply of pickups from room clear rewards.
    • Collectible Bird's Eye iconBird's Eye and Collectible Ghost Pepper iconGhost Pepper are exceptionally powerful combined with high luck: at 10 luck, every tear has a 50% chance to be accompanied by a powerful flame that blocks enemy projectiles. • Wooden CrossWooden Cross: A single-use Holy Mantle per floor. Note that it does not break before Holy Card, thus it is wise to save them for when the shield from Wooden Cross is broken.

Bad Items[ | ]

These items provide significant drawbacks to Tainted Lost.

  • Any explosive tear effect, such as Collectible Fire Mind iconFire Mind or Collectible Ipecac iconIpecac, greatly increases the chance of fatal self-damage. Since Collectible Host Hat iconHost Hat and Collectible Pyromaniac iconPyromaniac cannot naturally appear as Tainted Lost, there are very few ways to reliably avoid explosive self-damage.
  • Tear effects that destroy rocks (Collectible Sulfuric Acid iconSulfuric Acid, Collectible Terra iconTerra) and the Stompy effect make firing from cover impossible – all but removing a safe method for Tainted Lost to attack non-flying enemies.
  • Collectible Finger! iconFinger!: May destroy TNT accidently.
  • Collectible Little Baggy iconLittle Baggy: Holy Cards will no longer spawn.
  • Collectible IBS iconIBS: The effects of poop caused by damaging enemies is completely random, which can lead to an accidental death or mantle loss.
    • Butt bombs may hurt you if you aren't paying attention, gas clouds from stinky poop could explode in a chain back to you if thrown at fires, and farts generate a gas cloud that will explode if you are next to a fire, leading to an untimely death.

Neutral Items[ | ]

These items can provide both benefits and drawbacks to Tainted Lost.

  • Although items which provide temporary invincibility such as Collectible A Pony iconA Pony and Collectible Mars iconMars can allow Tainted Lost to negate damage, their effects are difficult to time, and could result in charging into an enemy and being damaged.
  • Collectible Buddy in a Box iconBuddy in a Box: The familiar's potential tear effects are the same as Collectible Fruit Cake iconFruit Cake, and as such have a small risk of carrying Collectible Fire Mind iconFire Mind for a floor.
  • Collectible Dead Sea Scrolls iconDead Sea Scrolls: Can cause items such as Collectible Anarchist Cookbook iconAnarchist Cookbook to trigger, potentially causing damage, but may also spawn a Holy Card via Collectible Deck of Cards iconDeck of Cards.
  • Collectible Experimental Treatment iconExperimental Treatment: HP is not a factor for Tainted Lost, so it may be included as one of the two stats which are reduced. However, it is more likely to be included as a stat increase, which has no effect.
  • Collectible Lemegeton iconLemegeton: Can give Tainted Lost very useful items, such as Collectible Birthright iconBirthright, allowing him to have infinite lives until the wisp is destroyed, but it can also give run-ending items such as Collectible Ipecac iconIpecac. Because Lemegeton's effect is random, this is completely out of the player's control. If a very bad wisp is gained using Lemegeton, it is recommended to destroy it as soon as possible, but be cautious as this could result in taking damage.
  • Collectible Monster Manual iconMonster Manual: Can spawn Collectible Bob's Brain iconBob's Brain or Collectible Lil Delirium iconLil Delirium, which may cause unavoidable damage to the player. However, it can spawn many useful familiars too.
  • Collectible Number One iconNumber One: Decreasing maximum range exposes Tainted Lost to danger, but increasing fire rate may make it easier to defeat enemies.
  • Collectible Teleport! iconTeleport! / Collectible Teleport 2.0 iconTeleport 2.0: Can teleport Tainted Lost away from a dangerous room or before a fatal mistake is made. However, the player can teleport to a Curse RoomCurse Room (or to a Secret Room whose only opened exit is a Curse Room) which is inescapable without a Holy Card.

Unlockable Items[ | ]

Trivia[ | ]

  • While items like Collectible A Dollar iconA Dollar, Collectible Knife Piece 1 iconKnife Piece 1, and Collectible Knife Piece 2 iconKnife Piece 2 are not considered items that the Tainted Lost can find, they will still appear for him if the conditions for finding them are met.
  • While Harbingers will still drop Collectible Ball of Bandages iconBall of Bandages or Collectible Cube of Meat iconCube of Meat, Boss C.H.A.D. ingameC.H.A.D. will not drop Collectible Little C.H.A.D. iconLittle C.H.A.D..
  • If invincibility is used to play Blood Donation Machines, they can still produce a Collectible Blood Bag iconBlood Bag as a reward.
  • The achievement for unlocking Tainted Lost is "The Baleful", a possible title for the character.
    • "Baleful" means "ominous" or "harmful or malignant in intent or effect", possibly referring to how Tainted Lost has higher base damage than his regular counterpart and can find better items.
    • A more archaic meaning of "Baleful" is "Something/Someone wretched or miserable", and considering how Tainted Lost is incapable of using defensive heavy builds, being constantly at the mercy of enemies and dying usually at two hits, fit with this use of the word.
    • "The Baleful" is Tainted Lost's character name in Four Souls.
  • Before the 1.7.5 update, it was possible for Tainted Lost to find Collectible Yuck Heart iconYuck Heart and Collectible Best Bud iconBest Bud, despite these items being useless for the character.
    • Additionally, Collectible Heartbreak iconHeartbreak could be found and utilized to essentially gain 4 extra lives and extra damage, but was removed from the tag list.
  • Tainted Lost's unlock drawing could be a reference to a transition nightmare Isaac can have, depicting The Lost talking to Boss Mom ingameMom from the attic.
  • Tainted Lost unlocks 0 - The Fool?0 - The Fool?; the character can be linked to both the numeration and meaning of the Fool.
    • The Fool's numeration can be either 0 the lowest trump or 22 the highest trump, linked to his attributes — better items, no mantle, no health — as Tainted Lost will die in a single hit at any point, but can also build the strongest runs of any character because of those same attributes.
    • Reverse fool means apathy and nullity; this approach is often recommended to face Tainted Lost's runs because of how demanding the character is and how easy it is to lose these runs despite how powerful the character can become.
    • In a literal reversal of "the beginning of the journey" which The Fool represents, Tainted Lost represents the complete end of Isaac's existence, after his own mortal remains has vanished and even his own soul erodes into dust and cobwebs along with the house and material objects that hold his last remnant memories.
  • Tainted Lost's starting health is listed as "?" on the character selection screen.
