Skatole is a passive item.

Effects

• This item causes many Fly enemies to alter their behavior toward Isaac.
  • The following flies transform into a neutral normal Fly: Attack Fly, Dart Fly, Ring Fly, Army Fly, and Swarm.
  • The following flies no longer shoot at Isaac: Pooter, Super Pooter, and Tainted Pooter.
    • These don't deal contact damage, even without Skatole.
  • The following flies move slower and no longer deal contact damage: Level 2 Fly and Bulb.
    • Bulbs will not drain active item charges if touched.
  • The following flies no longer deal contact damage but retain their speed: Eternal Fly, Moter, Hush Fly, Ultra Famine Fly, Ultra Pestilence Fly, and Ultra Pestilence Fly Ball.
    • Moters will not split into Attack Flies, Eternal Flies will not turn into Attack Flies.
  • The following flies move slower and no longer deal contact damage, but can still damage Isaac through other means: Full Fly, Boom Fly, Red Boom Fly, Drowned Boom Fly, Dragon Fly, Dragon Fly X, Bone Fly, Sick Boom Fly, Bloodfly, and Tainted Boom Fly.
    • Bone Flies will still throw bones at Isaac.
  • The following flies no longer deal contact damage, but retain their speed and can still damage Isaac through other means: Sucker, Soul Sucker, Spit, Ink, Level 2 Willo, and Mama Fly.
  • The following flies move slower but will still deal contact damage: Boom Fly, Red Boom Fly, Drowned Boom Fly, and Fly Bomb.
  • The following flies are completely unaffected, retaining their normal attacks and speed: Swarm, Hush Fly, Sucker, Soul Sucker, Spit, Willo, Eternal Fly Bomb, and Tainted Sucker.
    • Willos and Tainted Suckers don't deal contact damage, even without Skatole.

• This item belongs to the Beelzebub set. Collecting three items from this set will transform Isaac into a giant humanoid fly.
• This item belongs to the Oh Crap set. Collecting three items from this set will transform Isaac into a walking pile of poop.

Notes

• This item can only be naturally obtained from the Shell Game.
  • Skatole exists in an otherwise unused shellGame item pool. This means it can also be acquired with Chaos and bosses in The Void can drop it.
• Beelzebub's effect overrides Skatole.
• Flies spawned by The Husk will not become passive.
• Due to being quality 0, Skatole cannot be obtained from Shell Games after Sacred Orb has been picked up.
• This item makes The Duke of Flies a lot easier, as most of his attacks, which spawn flies, will instantly be converted. Staying out of his way while having Skatole can even make it so he ends up killing himself.

Trivia

• Skatole is a mildly toxic white crystalline organic compound that occurs naturally in feces.
• According to Edmund McMillen's First post-Repentance Lore Stream, Skatole's visual was designed first, with McMillen then working backwards to create an appropriate effect to justify an item that puts a poop on Isaac's head.
• Skatole was considered as a cosmetic hat for Team Fortress 2, alongside tears, Brimstone, and Tiny Planet. It was dropped as Valve "... didn't want players running around with a wet piece of poop on their heads."
• This item is used in Scat Man (challenge #36).
• This item is one of the only five that count towards 2 transformations, the other four being Hive Mind, The Bible, Magic Mushroom and Parasitoid.

Bugs

Bug!	Skatole's slowdown effect also affects Tumors and its variants, despite them not being flies.
