Sheol is one of two unlockable floors following the WombWomb/UteroUtero/(except in Rebirth)Scarred WombScarred Womb, the other being the CathedralCathedral. After defeating Boss Mom's Heart ingameMom's Heart, a devil room may spawn containing a trapdoor leading down to Sheol. Unlocking and defeating Boss It Lives ingameIt Lives will always spawn a trapdoor leading to Sheol, as well as a Beam of Light leading to the Cathedral. Sheol can also be accessed using Collectible We Need to Go Deeper! iconWe Need to Go Deeper! from The Womb II, even if Mom's Heart has not been defeated. It can also be entered if a JokerJoker is used on the Womb II, as it may bring the player to the Devil RoomDevil Room with a trap door.

Sheol has only one floor, featuring undead and dark/demonic enemies, and the boss is always Boss Satan ingameSatan.

  • In Hebrew, Sheol is translated as "place of the dead"; it is described as the common afterlife of humans in Jewish religious lore (including the Old Testament), making it the Abrahamic version of the underworld.
  • Sheol is the ninth level of the game (without there being a Curse of the Labyrinth). The 14th-century novel Inferno, the first part of Dante Aligheri's famous Divine Comedy and one of the first books to describe Hell in detail, notes that the ninth and lowest layer of Hell, where those who commit the sin of betrayal are sent, is frozen. On Isaac's last will, it says he died in "a cold place" if he dies in Sheol.
    • The ninth layer of Hell is said to be where Satan, who turned against God and was cast down for it, is imprisoned, indicating the reason for Satan's presence in the area.
    • It's also said that the ninth layer of Hell is the prison for Judas Iscariot, the betrayer of Jesus, which may be the reason that Character Judas iconJudas is unlocked there.
  • Like in all chapters, small non-interactive objects can be found on the ground. One of these decorations is a drawing of a triangle that may be a reference to the Triforce, an iconic relic from The Legend of Zelda series.
  • There is an unused version of Sheol, which uses textures for Dark Room, but with a black background. It can be accessed by typing stage 10b in the Debug Console.
