- Grants one
Black Heart. - Activates The Necronomicon effect upon taking damage if Isaac's total health ( Red Hearts, Soul Hearts, and Black Hearts) is at one heart afterward.
- Deals 80 damage to everything in the room upon taking damage if Isaac's total health (Red Hearts, Soul Hearts, and Black Hearts) is at one heart afterward.
- Adds +40 damage to The Necronomicon and similar effects, such as Black Hearts.
Synergies[]
- Black Feather: Counts as an 'evil' item, granting a damage up.
- Blood Rights / Dry Baby / The Negative / XIII - Death: The damage boost is applied to these items.
- A Missing Page/Missing Page 2: The damage increase from the trinket/copies of Missing Page 2 stacks, adding +40 damage per stack to The Necronomicon and similar effects.

- Works the same for Keeper and Tainted Keeper and his Coin Hearts.

- The Necronomicon, A Missing Page, and Missing Page 2 are all references to the Necronomicon and its missing pages appearing in Evil Dead. Which, in turn, is a reference to The Necronomicon, a fictional grimoire appearing in the works of horror writer H.P. Lovecraft.
