King Baby is an unlockable passive item added in The Binding of Isaac: Afterbirth †.

Effects[ | ]

  • Grants a familiar that follows Isaac and always appears first in the familiar chain.
  • While firing tears, King Baby stops moving and glows white.
  • When Isaac stops firing tears, King Baby moves rapidly back towards Isaac.
  • While King Baby is stationary, other familiars remain stationary as well. (in Repentance) This affects every familiar that has priority in how closely it follows behind Isaac.
    • (except in Repentance) Shooting familiars will fire tears in the direction Isaac is firing, while Throwable familiars ignore King Baby.
    • (in Repentance) Shooting familiars fire at nearby enemies automatically and throwable familiars launch themselves at nearby enemies automatically.
      • Familiars that have charged attacks will operate on similar rules to Collectible Marked iconMarked: they will immediately fire at full charge, and variable-charged attacks will be released earlier the closer that King Baby is to the target.
  • (in Repentance) This item belongs to the ConjoinedConjoined set. Collecting three items from this set will transform Isaac into a three-faced version of himself.

  • King Baby will move back towards Isaac during Isaac's hurt animation, even if a fire button is still being held.
  • Collectible Abel iconAbel will still reflect Isaac's movements, and not King Baby's.
  • Collectible Lil Delirium iconLil Delirium's behaviour depends on which familiar form it takes.
  • Duplicate King Babies, such as those granted from Collectible Box of Friends iconBox of Friends, will cause familiars to follow the last King Baby added.
  • (in Repentance) Familiars that follow behind Isaac have a certain priority in how close their position is to him over other familiars, with King Baby having 1st priority and thus being closer to Isaac than familiars of different priorities.
  • (in Repentance) Has no effect on Character Tainted Eve iconTainted Eve's/Collectible Sumptorium iconSumptorium clots.

  • (except in Repentance)Collectible Isaac's Heart iconIsaac's Heart: Isaac can fire tears and leave King Baby and the heart behind, and run into a group of enemies without being damaged.
  • Collectible Lil Brimstone iconLil Brimstone: Can fire in full 360 degrees and will fire as soon as he's fully charged, making him extremely effective.

  • Child LeashChild Leash: Familiars stay closer to King Baby while Isaac is firing.
  • (in Repentance)Collectible Cain's Other Eye iconCain's Other Eye: Cain's Other Eye will attempt to fire directly at enemies but only when they are on the same side as the random direction it is firing in.
  • Duct TapeDuct Tape: King Baby will stay in place relative to Isaac until Isaac starts firing tears. Then King Baby will remain stationary until Isaac has stopped firing and will rapidly move back to him.
  • (in Repentance)Friendship NecklaceFriendship Necklace: Familiars will orbit King Baby at all times instead of Isaac, even while not firing. King Baby itself is unaffected.
  • (in Repentance)Collectible GB Bug iconGB Bug: GB Bug will only throw itself at nearby enemies automatically if a fire button is double tapped while King Baby's effect is active.
  • Collectible Gemini iconGemini: Suture will rest near King Baby as long as there are no enemies near Isaac. When Isaac gets near enemies, Suture will rapidly snap from King Baby toward the enemy. Functionally, King Baby has no real effect on Gemini.
    • Visually, this can result in the umbilical between Isaac and Suture getting stretched to a long length.
  • (in Repentance)Collectible Jaw Bone iconJaw Bone: After being thrown, Jaw Bone will return to Isaac and then reappear behind King Baby.
  • (in Repentance)RC RemoteRC Remote: Overrides King Baby's halting; familiars will mimic Isaac's movements even while firing. King Baby's auto-targeting remains active.

  • King Baby references the official Binding of Isaac tarot card set, in which Character ??? (Character) icon??? represents the Emperor.
  • The Steam achievement description for unlocking King Baby says "Unlocked a new co-player baby," despite it being an item.
  • Before Repentance, King Baby's pickup quote was "Lord of the dead!".
  • (in Repentance) King Baby's pickup quote is a reference to the film Army of Darkness, where the protagonist Ash says "Hail to the king, baby!".
