Effects are one major mechanic of Binding Of Isaac: Rebirth. There are a number of different items that alter the type of tears and the status effects they inflict.

Template:DisambigMsg Tear Effects modify the player character's primary weapon — tears. The player's damage, rate of fire, and range are among the attributes further affecting the behavior of their attacks. Generally, familiars and co-op partner babies also use tears, and may have inherent tear effects of their own. Any item that the main character picks up will also affect any baby controlled by a co-op partner, but familiars' tears are not granted the effect.

Bouncing Tears

Bouncing tears ricochet off of walls, obstacles, and enemies upon contact, rather than bursting. Tears that hit enemies bounce off in random directions, but tears that hit walls or rocks will go directly in the opposite direction they came in.
As tears do not disappear on contact, the only way bouncing tears can disappear is by running out of range and hitting the floor.

Charged Tears

Charged tears replace the character's auto-fire ability and instead require the player to hold the fire button and then release it to shoot. There are a few items which provide charged tears, and the effect varies between all of them.
While charged tears provide an increase in instant damage, the auto-fire removal usually generates a significant drop in overall damage per second.
Charged tears should not be confused with tear-replacing items which require charging. These items do not replace Isaac's tears.

Charming tears can inflict the charm status effect, which causes enemies to temporarily fight on Isaac's side, running toward other enemies and damaging them.

Concussive Tears

Concussive tears can inflict the confusion status effect, which causes enemies to walk around randomly.

Infestation Shot

Infestation shot gives tears the ability to spawn friendly blue spiders or blue flies that deal extra contact damage to enemies.
While there are multiple items that can spawn blue spiders or blue flies, only two items add that effect to tears.
Mom's Wig does not give infestation shot, but the spiders are generated only when the character is firing, and thus the item is included in this category.

Magnetic Tears

Entities, including monsters, pickups, and trinkets, are magnetically attracted to the player's tears. Entities are attracted dependent on the distance from each tear. Different enemy types are accelerated at different rates.

Multiple Shot

Multiple shot replaces the character's alternating shot of tears from the right and left eye with a single shot of multiple tears, usually at the cost of a rate of fire decrease.

Petrifying Tears

Petrifying tears can inflict the petrified status effect, which stops enemies in place and prevents them from moving or attacking.

Piercing Tears

Piercing tears travel through enemies (but not obstacles) instead of breaking on impact with them. While they do not directly increase damage, piercing tears can allow the character to deal a greater amount of damage per second due to the multiple impact calculations on large bosses and the possibility to hit multiple enemies at once.
The downside of piercing tears is that they remove knock-back.

Poison Tears

Poison Tears deal poison damage over time. Poisoned tears have a 100% chance to inflict the status effect on regular enemies, but bosses have a chance of not being poisoned.

Rainbow Tears

Rainbow tears are not a specific type of tear. Instead, a character or familiar which has been granted rainbow tears will randomly fire any other type of tears.

Shielded Tears

Shielded tears can block enemy shots upon collision. This destroys the shielded tear unless it is also a piercing tear.

Shrinking tears shrink enemies, causing them to run away from Isaac. Isaac can step on shrunken enemies to kill them.

Spectral Tears

Spectral tears travel through obstacles (but not enemies) instead of breaking on impact with them. Spectral tears can be used to safely dispatch most types of monsters from behind obstacles, or over them in combination with flying.

Slowing tears can inflict the slow status effect, which reduces the enemies' movement speed, their shot speed and range, and impedes their charge attacks.

Tear-replacing items

The following items are not considered tears, and instead replace them. While they do benefit from increases to stats, like damage or rate of fire, they usually override most tear-enhancement effects, homing and status effects being among the few exceptions.

Status Effects are special effects that can be inflicted on an enemy from a variety of sources, such as tears, bombs, and items. Each status effect has a colour and symbol/effect associated with them for easy identification. These colourations are not to be confused with champions.

Status Effects

Effect Appearance Description
Burn Burn Effect Afflicted enemies take fire damage over time, like poison. Afflicted enemies will be tinted orange for the duration and have a fire symbol above them.
Charm Charm Effect Afflicted enemies will temporarily fight on Isaac's side, running toward other enemies and damaging them. If no other enemies are in the room, a charmed enemy will instead run towards Isaac. (Charmed enemies can still hurt Isaac.) Afflicted enemies will be tinted pink for the duration and have a heart symbol above them. Most enemies with ranged attacks will not fire them.
Confusion Confusion Effect Afflicted enemies are slowed and walk around randomly. While confused, most non-boss enemies will be unable to attack. Afflicted enemies will be tinted pale gray and have stars above them.
Fear Fear Effect Afflicted enemies are scared of Isaac and will run away from him. While scared, most non-boss enemies will be unable to attack or charge towards Isaac. Afflicted enemies will be tinted purple and have a skull symbol above them.
Petrified Stop Effect Afflicted enemies are stopped in place. They cannot move or attack. Afflicted enemies will be shaded brown-gray, as if turned to stone (unless petrified by Midas' Touch, in which case they will be shaded gold).
Poison Poison Effect Afflicted enemies sustain damage over time. Afflicted enemies will be tinted green and have a bubbly effect surrounding them.
Slow Slow Effect Afflicted enemies are slowed down. Any projectiles they shoot will also be slowed down. Attacks that end when the enemy runs out of momentum, such as the charging attacks of Gurdy Jr. and Dingle, will be vastly shortened in duration. Afflicted enemies will be tinted white and have a snail icon above them.

Gaper Afflicted enemies are shrunken and will run away from Isaac. Isaac can walk over shrunken enemies to kill them.

Temporary buffs are beneficial effects applied to the player character that will expire after a timed duration or on leaving a room, and are not tear effects.

Attribute Buff

An attribute buff increases the value of the player's stats. Damage, tears, and health are among those affected. As with other temporary buffs, the effect will expire.

Invulnerability

Invulnerability negates all types of damage for five seconds, including health loss from Blood Donation Machines and Devil Beggars, environmental hazards like spikes, and Curse Rooms doors and Sacrifice Rooms, but does not prevent heart container or Soul Heart loss from Health Down Pills, Devil Room deals, or spawning a baby.
While under the effects of invulnerability, the character has increased speed, loses the ability to shoot tears, flashes in multiple colors, and the music of the game speeds up. However, all invulnerability-granting items except for the Unicorn Stump also grant the character the ability to damage enemies by contact for the duration of the effect.
