A Cod Worm is a special variant of the 
Para-Bite in an immobile shell that can be found in Chapter 4. It was introduced in the vanilla content of The Binding of Isaac: Rebirth.
Instead of chasing Isaac like the regular Para-Bites, the Cod Worm stays in its shell and will spew a burst of blood shots if shot at. After some time, it will reveal itself and become vulnerable to attacks.
