Rebirth features 2 player local co-op on PC. The Afterbirth DLC increased this to 4 player local co-op. The PlayStation 4 and Steam versions of the game are capable of both local and online multiplayer via PS4's "Share Play" and Steam's "Remote Play Together" features, as well as a dedicated online multiplayer system in early beta. (These versions can also support up to 4 players.)

Co-op players can join in at any time as a random baby, taking a Red Heart container away from Isaac and giving it to the second player. Subsequent pressing of the join button will despawn the baby, giving Isaac the Red Heart container back.

Babies share Isaac's stats except for health and damage; babies deal half of Isaac's damage. Babies always have flight, but they cannot interact with objects like items and pickups, except for certain special babies like Magnet Baby and Isaac 2.

Joining as a co-op baby will bring up a menu that allows the player to pick from 5 special babies. Dying as one of the available special babies will remove it from the pool.

If you start a normal game and player 2/3/4 presses Start while in the starting room, a mini character selection wheel displays at the top of the screen, with which that player can choose any unlocked character, or a random character. If a player dies without any revival items, they become a ghost baby that can only deal minimal damage and press buttons. Once the boss of a floor is killed, the dead player is reborn with all the items they once had, but with half a heart. When a boss is killed, the item pedestal will spawn as many items as there are players. All players share resources (coins, bombs, and keys), but each have their own hearts and items. All damage players do to bosses is reduced by 20%, including the damage from items, familiars, or bombs. Once all players die, it's game over.

Notes (Co-op Babies)[ | ]

Soul Hearts will be given to Player 2 before normal hearts, and half hearts will be given before full hearts. If player 2 dies, they can rejoin the game at the cost of another Red Heart container, as a new random Baby.
The appearance of a majority of the co-op babies is solely cosmetic, but there are several "special" babies with beneficial effects. However, some special babies have detrimental effects.
The last shooting familiar Isaac picked up will follow Player 2 instead of Isaac.
If Isaac has more than 6 total hearts, including Soul Hearts and Black Hearts, Player 2 will take two hearts instead of one.
If Isaac has a maximum of 12 hearts, including Soul Hearts and Black Hearts, Player 2 will take three hearts instead of one or two.
The maximum amount of hearts Player 2 can have is three Red Hearts and one Soul Heart, using Scapular.
- This limit increases by one Red Heart container, Soul Heart, or Black Heart when using Red Baby, Blue Baby, or Black Baby, respectively.
Because at least 1.5 hearts are required for co-op, Player 2 cannot join when playing as The Lost.
On the Nintendo Switch, you must restart the game after setting up the controllers or the join button will not work.

Strategy (True Co-op)[ | ]

Magdalene not only heals herself with Yum Heart, but also heals all other co-op players for a Half Red Heart. It's best to pair Magdalene up with characters who benefit or rely heavily on red health, such as Samson, The Forgotten, or Bethany.
Cain can pair well with Lazarus, as Cain can take Lazarus' pill to have it be a guaranteed positive or neutral pill for Cain.
- Lucky Foot will not convert pills unless Cain holds them, so it can still be a negative pill if used by Lazarus.
Tainted Lost's reroll mechanic will apply in co-op, but this still includes the reroll of any items not labelled as "offensive".
- Since there will be significantly less health-up sources as a result, it's recommended to pair Tainted Lost with characters who can't use red health, like ???, Keeper, Tainted Judas, Tainted ???, Tainted Keeper, Tainted Forgotten, or Tainted Bethany. Tainted Cain can also work, since his Bag of Crafting entirely bypasses the reroll mechanic.
Keeper/ Tainted Keeper will not replace heart drops with flies when paired with other characters that use hearts.
On kill effects such as Infestation 2, Lusty Blood, or Tainted Keeper's coin drops will trigger for the player holding it regardless of which player killed the enemy.
- Similarly, effects that trigger on damage, such as It Hurts trigger on players who take damage, regardless of who took damage.
Tainted Magdalene synergizes with Tainted Bethany as due to Tainted Maggy's generation of heart pickups, Tainted Bethany can quickly accumulate Blood Charges.
Tainted Isaac can be a viable choice for a character, as the changed item pedestals benefit all players, not just Tainted Isaac.
Due to the overall lack of items, visiting Curse rooms becomes a lot more viable, especially since one can play as a character that mitigates damage from Curse Rooms, such as Tainted Judas with his innate Dark Arts.
Similarly to Jacob and Esau, if you time it properly, both items can be acquired from alternate path treasure rooms.

45	Dripping Baby	Occasionally briefly gives Isaac Anemic, increasing range by 5 and causing Isaac to leave a trail of blood that damages enemies. Earn all Hard mode Completion Marks as Lazarus to unlock it.
46	Blinding Baby	Upon death, activates the effect of XIX - The Sun, fully restoring Isaac's red hearts, deals 100 damage to all enemies in the room, and revealing the map for the current floor. Use Blank Card while holding XIX - The Sun to unlock it.
47	Sucky Baby	Has an aura around itself that damages enemies while they are inside, similar to Succubus. Earn all Hard mode Completion Marks as Azazel to unlock it.
48	Dark Baby	Occasionally provides a brief boost to all of Isaac's stats. Earn all Hard mode Completion Marks as Lilith to unlock it.
49	Picky Baby	Occasionally briefly gives Isaac the Paper Clip effect. Earn all Hard mode Completion Marks as Cain to unlock it.
50	Revenge Baby	Occasionally briefly increases Isaac's damage. Earn all Hard mode Completion Marks as Samson to unlock it.
51	Belial Baby	Occasionally briefly gives Isaac the The Book of Belial effect, increasing damage. Earn all Hard mode Completion Marks as Judas to unlock it.

The familiar uses Isaac's tears, range, shot speed, and tear effects, and deals 0.5x Isaac's damage. The familiar has collision and can be separated. Holding down the "drop" button keeps the familiar in place. When Isaac places a bomb, the familiar will drop one as well. The familiar can also press Buttons. The familiar dies after taking any damage, and will respawn on the next floor.
60	Baby Lost White	A co-op player turns into this after dying. After the floor's boss is defeated, the player reverts to normal. It cannot fly over pits but does not take damage. Can fire very weak tears.

Possessor[ | ]

There is a hidden multiplayer game mode that allows a second player to control enemies and bosses in a way similar to Friend Finder. To access it, the second player should hold both bumpers on their gamepad and press the Start/Options button (On keyboard, the second player should hold Q and E, then press Enter). After this, a message saying that a player has joined as the possessor will appear in the Debug Console. It works properly for most normal enemies and some specific bosses, such as Ultra Greed, but for the rest, controls are limited or do not work at all.

Notably, all of Bethany's unlockable co-op babies made it in the Repentance DLC along with Double Baby but not the others (the final 3 being that Edith was scrapped as a character altogether for Antibirth and would never return as a realized concept).

Bug!	PC If the co-op player joins using a keyboard, the join button and the fire button must be pressed simultaneously to bring up the special baby menu. It may take several tries, as the timing seems to be almost frame-perfect.
