The Cathedral is one of two floors following the Womb/Utero/Scarred Womb, the other being Sheol. After defeating Mom's Heart, depending on Isaac's deal chance, an Angel Room may spawn which will only contain a beam of light leading upwards to the Cathedral. Other methods to enter the deal room such as Joker or Red Chests will also work. Unlocking and defeating It Lives will always spawn a beam of light leading to the Cathedral, as well as a trapdoor leading to Sheol.
The Cathedral has only one floor, featuring mostly undead and/or holy enemies, and the boss is always Isaac.
Blue Fire Places are far more common in this floor than normally.

Trivia[]
- In the original Binding of Isaac, the windows along the walls were much larger and had different patterns resembling Isaac and a crucifix.
- The floor's theme, Everlasting Hymn, is one of the few tracks, together with Morning Star, Living in the Light and Revelations 13-1, that contains actual lyrics: Dei, quod reliquum est, Nolite metuere, Et ossa tua liberabit, Exspectans.
- Even though the latin in the song likely contains a few mistakes (like dei, which can only mean "of god" or "the gods" as subject of the sentence, or the inconsistency between the singular vocative "expectans" and the plural verb "nolite metuere") the more accurate translation would be: "O thou who await, ye shan't fear what is left of God, and He will set thy bones free." Considering the fact that Isaac believes that killing himself is the morally correct thing to do (as he sees himself as being sinful or corrupted), the lyrics are encouraging Isaac to stay in the box and suffocate as his body panics for air.
- The theme of Cathedral (Everlasting Hymn), is noticeably shorter than other floor themed soundtracks. The second layer of the theme (for when entering a room with enemies and bosses with a high HP count), wasn't added in the official Ost, with the in-game second track being barely noticeable chants. The Dark Room's theme (Devoid) is also shorter, but actually lacks the second, intense part of the track.
- Interestingly, Cathedral was the only non-final level with no unique enemies. This was why it was given some enemies exclusive to the floor in Repentance (those being Holy Mulligans, Holy Eyes, and Holy Bonies).
- Holy Eyes, Holy Bonies, and Holy Mulligans were first seen in cards from the card game The Binding of Isaac: Four Souls.
